Empath Home Health, a member of Empath Health is seeking a compassionate Weekend Baylor RN Home Health (Registered Nurse) to work with our Home Health team to deliver individualized care to patients in their homes throughout Sarasota & Charlotte counties (Sarasota to Port Charlotte area).

Locations: Home health visits in Sarasota and Charlotte counties (North Sarasota, Venice, Port Charlotte, North Port, and surrounding areas)
What You’ll Do
- Perform comprehensive patient assessments, including OASIS assessments at required intervals
- Develop, initiate, and revise individualized plans of care as patient needs evolve
- Deliver skilled nursing services and treatments as outlined in the plan of care
- Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
- Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need
- Graduate of an approved nursing program with an active Florida RN license
- Prefers BSN or MSN degree
- Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred
- Prefers experience using Homecare Homebase (HCHB) EMR
- Strong assessment, critical thinking, and patient education skills
- Willingness to participate in weekend and holiday on-call rotations
