What You'll Do
Coordinates patient admissions activities in the long term care facility, patient home, hospital, assisted living facility and hospice house settings.
- Educates patient, family and caregivers regarding disease process and decline, palliative interventions, care giving, dying process, ethical concerns and safety practices.
- Provides initial assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living.

- Must possess a clear and current Florida Licensure as RN. BSN or MSN preferred.
- Minimum one year experience in an acute-care Hospital / In-patient environment providing direct patient care; previous experience as a Hospice RN preferred.
