Registered Nurse RN Home Health

Empath Home Health, a member of Empath Health is seeking a compassionate Home Health RN (Registered Nurse) to work with our Home Health team to deliver individualized care to patients in their homes in Sarasota County.

Locations: Home health visits in Sarasota

Schedule: M-F 40 hours. Required weekend/on-call/holiday rotations.

What You’ll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need 

• Graduate of an approved nursing program with an active Florida RN license
• Prefers BSN or MSN degree
• Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred
• Prefers experience using Homecare Homebase (HCHB) EMR
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
