What You'll Do
- Provide ongoing monitoring of physical, psychological, social and spiritual needs of the Patient/Family/PCG and initiates appropriate intervention or referral in collaboration with Primary RN.
- Provide accurate and appropriate documentation of patient/family services as provided for in Empath Health policies and submit in a timely manner.
- Provide nursing care under the direction of the Primary RN to a specified group of patients, to provide continuity of care. Participates in weekly review and update of care plans in collaboration with Primary RN.
- Receive and implement orders under the direction of the RN from the physician.
- Following a patient’s death collaborates with primary RN/Counselor and participates as needed in closure contact with family/caregivers. Demonstrate knowledge of Empath Health policies and procedures and pertinent State and Federal Rules and Regulations.
- Administer medications and treatments prescribed by the attending physician in collaboration with Primary RN and/or PFCC.

- Licensure:
- Valid Florida LPN License.
- Valid Florida State Driver’s License.
- Certifications:
- Valid CPR certification from the American Red Cross or American Heart Association
- Current IV Therapy certification preferred. Must obtain the certification within six months of hire date.
- For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
