Responsibilities
- Administer medications and provide treatments as prescribed
- Conduct nursing assessments to monitor patient health status
- Assist patients with personal care activities, including bathing, grooming, and mobility
- Educate clients and families on medication management and health maintenance
- Document care provided and maintain accurate patient records
- Communicate effectively with the healthcare team and act as a liaison between patients, families, and providers
- Maintain a safe and supportive environment for clients
- Active LPN license in the state of Connecticut
- Previous experience in home care or similar setting preferred
- Strong clinical assessment and nursing skills
- Excellent communication and interpersonal skills to engage with clients and families
- Ability to work independently and manage time effectively
- Reliable transportation and valid driver's license
- Must have current CPR certification
- Flexible schedule to accommodate varied per diem shifts
