Eminence Home Care is currently seeking a compassionate and dedicated Part-Time Home Care Assessment LPN to join our team. In this crucial role, you will conduct assessments for clients in their home environments, collaborating with healthcare professionals to create personalized care plans that meet each client's specific needs. Your responsibilities will include evaluating clients’ health status, documenting findings accurately, and providing essential education to clients and their families about managing health conditions. Responsibilities
- Perform initial assessments of clients to determine their medical and caregiving needs.
- Collaborate with RNs and other healthcare team members to develop comprehensive care plans.
- Monitor and document patient status, ensuring all information is accurate and up to date.
- Provide education and support to clients and families regarding health management and care strategies.
- Ensure compliance with all healthcare regulations and company policies.
- Communicate effectively with clients, families, and the healthcare team to promote seamless care transitions.
- Current LPN license in the state of Connecticut.
- Previous experience in home care, nursing, or a related field preferred.
- Strong clinical and organizational skills.
- Effective communication and interpersonal skills for working with diverse populations.
- Ability to work independently and as part of a healthcare team.
- Reliable transportation and a valid driver's license. Must be willing to travel!
- Current CPR certification is required.
