As part of the clinical team, you'll provide comprehensive primary care while working closely with physicians, clinical staff, and other healthcare professionals.
Responsibilities Include
- Evaluate, diagnose, and treat patients across multiple age groups
- Perform routine physicals and preventive health visits
- Develop individualized treatment plans
- Diagnose and manage acute and chronic health conditions
- Prescribe medications within your scope of practice
- Order and interpret appropriate diagnostic testing
- Provide preventive care and health education
- Educate patients on chronic disease management, nutrition, medications, and healthy lifestyle choices
- Coordinate referrals and follow-up care when specialty services are needed
- Collaborate with physicians and other members of the clinical team
- Maintain accurate and timely clinical documentation
- Support continuity of care and help patients navigate healthcare services
- Participate in clinical quality and patient-care initiatives
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in New York
- Strong primary care or related clinical experience
- Comfortable providing care to patients across multiple age groups
- Strong assessment, diagnosis, and treatment skills
- Comfortable managing both acute and chronic conditions
- Strong patient education and communication skills
- Interest in community-based healthcare
- Ability to work collaboratively within a multidisciplinary clinical team
