What are the responsibilities and job description for the Direct Primary Care Nurse Practitioner position at Gem State Direct Primary Care?
Nurse Practitioner — Part-Time with Opportunity to Grow to Full-Time
Gem State Direct Primary Care | Hayden, Idaho
Gem State Direct Primary Care is seeking a personable, motivated nurse practitioner to join our growing, membership-based practice. This position will begin part-time and expand toward full-time as the provider builds an established patient panel, with a long-term goal of approximately 300–350 patients.
We value meaningful patient relationships, thoughtful care, and a workplace where we can support each other, laugh, and enjoy what we do. Our approach combines evidence-based primary care with individualized functional and complementary approaches, including homeopathic modalities when appropriate, with attention to safety, available evidence, and patient preferences.
About the Role
This is an opportunity to build your own patient panel within a small, collaborative practice. You will provide comprehensive primary care, develop lasting relationships, and help patients navigate both immediate concerns and long-term health goals.
Because we are a small clinic, this role requires independence, flexibility, and a willingness to wear many hats. Medical assistant support will initially be available as needed. During the early stages of panel growth, the provider should expect to perform most routine clinical and administrative tasks independently. Dedicated support will be reassessed when the panel reaches approximately 100–150 patients, based on workload and practice resources.
Responsibilities
- Provide preventive care, acute visits, chronic disease management, and individualized treatment plans.
- Manage an assigned patient panel and contribute to its growth through excellent care and strong patient relationships.
- Order and interpret testing, prescribe medications, coordinate referrals, and communicate results.
- Complete timely documentation and manage patient messages, refills, and follow-up.
- Perform routine hands-on clinic tasks, including rooming patients, obtaining vital signs, and collecting specimens as needed.
- Participate in weekday on-call coverage and respond to urgent patient texts and calls over the weekend according to the practice’s coverage schedule and response expectations.
- Collaborate with the owner and staff to maintain an organized, welcoming clinic and improve workflows.
- Pitch in wherever needed as part of a small team.
Who We’re Looking For
- A board-certified nurse practitioner with an active, unrestricted Idaho license and appropriate prescribing credentials.
- Strong clinical judgment and confidence managing primary care independently.
- Interest in evidence-based medicine, functional medicine, and thoughtful integration of complementary approaches.
- A warm, approachable personality, a sense of humor, and a positive attitude.
- Excellent communication, organization, and follow-through.
- Comfort with direct patient communication by text and phone.
- A dependable team player who takes initiative and is willing to handle responsibilities beyond patient visits.
Primary care experience is preferred.
Schedule and Work-Life Balance
The position will start part-time, with increasing hours as the patient panel and workload grow. We offer flexibility during the week and value a sustainable balance between work and personal life.
Direct patient access is central to our care model. Weekday availability and urgent weekend communication are required, with coverage arrangements and response expectations established before employment.
Compensation and Benefits
Compensation will include a starting base pay during panel development, with a transition toward compensation tied to assigned patient panel size and associated membership revenue as the panel grows. The compensation structure, growth milestones, and transition to full-time will be outlined in the employment agreement.
Employer-sponsored medical insurance is not currently offered.
We offer:
- Flexibility in the weekly schedule.
- A supportive, personable work environment.
- Time to build meaningful relationships with patients.
- Clinical autonomy with collaboration and support.
- An opportunity for compensation to grow alongside your patient panel.
- A meaningful role in shaping a growing local practice.
