About the Role:
As a Licensed Practical Nurse (LPN) with Emerald Hills, your primary responsibilities will include:
Providing direct patient care, including administering medications, performing wound care, and assisting with activities of daily living
Developing and implementing individualized care plans in collaboration with the healthcare team
Monitoring patient progress and reporting changes to the nursing staff and medical director
Maintaining accurate and detailed records of patient care and treatment
Collaborating with the healthcare team to ensure seamless transitions and referrals
Participating in quality improvement initiatives and professional development opportunities
Ideal Candidate:
Current LPN license in the state of New Jersey
1 year of experience in a long-term care or rehabilitation setting
Strong communication and interpersonal skills
