Job Summary:
We are seeking a dedicated and compassionate Case Manager 1 to work with young adult males in collaboration with the Department of Mental Health (DMH). The ideal candidate will have a minimum of two (2) years of experience in case management, specifically working with individuals who have experienced mental illness and various traumas. This role requires a deep understanding of trauma-informed care, crisis intervention, and community resource coordination.
Key Responsibilities:
- Provide comprehensive case management services to young adult males with mental health challenges.
- Develop and implement individualized service plans (ISPs) to support clients in achieving stability and personal growth.
- Collaborate with DMH and other community organizations to coordinate care and access to necessary resources.
- Conduct regular assessments to monitor client progress and adjust plans accordingly.
- Advocate for clients by connecting them with housing, employment, education, and healthcare services.
- Utilize trauma-informed approaches to support clients in navigating past and present challenges.
- Facilitate group sessions, life skills training, and crisis intervention as needed.
- Maintain detailed and timely documentation of client interactions and progress in accordance with agency and DMH guidelines.
- Ensure compliance with all state and federal regulations regarding client care and confidentiality.
Qualifications:
- Minimum of 2 years of case management experience, preferably working with young adult males.
- Experience working with individuals with mental illness and trauma histories.
- Knowledge of DMH services and community-based mental health resources.
- Strong understanding of trauma-informed care and crisis intervention techniques.
- Excellent communication, advocacy, and problem-solving skills.
- Ability to work both independently and collaboratively in a team-oriented environment.
- Proficiency in case documentation and electronic record-keeping systems.
- Bachelor’s degree in Social Work, Psychology, Human Services, or a related field (preferred but not required, depending on experience and certifications).
Preferred Qualifications:
- Experience in housing navigation, employment support, and substance abuse counseling.
- Bilingual skills (Spanish) are a plus.
- Familiarity with court-involved youth or reentry programs.
