Key Responsibilities

  • Provide comprehensive, integrated, recovery oriented, on-site case management services to Transitional Age Youth clients (TAY) from ages 18-25 under the direction of Executive Director and Operations Manager.
  • Conduct independent living skills and recovery-oriented groups as scheduled.
  • Provide intensive crisis intervention services to members at risk of hospitalization and /or incarceration in an effort to avert hospitalization and /or incarceration.
  • Maintain at least weekly scheduled case management service meeting with all assigned clients on caseload.
  • Prepare for weekly clinical staff meetings by having current information on the status of all assigned members.
  • Track client's medication to ensure that they are medication compliant and that they have an adequate supply of medication at all times.
  • Provide mental health rehabilitation services, such as, counseling, money management, social skills training, and other independent living skills as needed by assigned clients.
  • Develop and maintain Community Resource book targeted to TAY resources
  • Act as Lead team member providing direction to Resident Aides in the care of TAY and other populations housed at our facilities.
  • Facilitate and support clients’ movement throughout the program with particular attention to need of ancillary resources and progress toward meeting goals outlined in treatment plans.
  • Facilitate orientations, groups and coordinates care with client and key stakeholders, assists in providing client job and vocational referrals and support clients at external appointments.
  • Participate in development of discharge plans and levels of care including extension requests based on need.
  • Attends required trainings and meetings.
  • Maintains accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by Ella’s Foundation.

Education and Knowledge, Skills and Abilities

Education Required:

  • Must have at least five years of experience working in the field of mental health / social services with a HS Diploma.

or

  • Must have at least four years of experience with an AA degree in the field of mental health / social services.

or

  • Must have at least two years of experience with a BA degree in the field of mental health / social services.

  • SPECIAL SKILLS AND KNOWLEDGE:

Desired:

  • Certification or final stages of certification with a Drug and Alcohol Certification program recognized by DHCS
  • Proficient with HMIS, Clarity, CES systems
  • Must not be on active parole or probation

Skills and Abilities Required:

  • Knowledge of mental health and substance abuse recovery principles and resources; including Co-occurring disorders and trauma informed treatment
  • Knowledge of principles and techniques of social work, case management, crisis intervention, client centered counseling, and group facilitation
  • Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications
  • Ability to relate to individuals who have severe impairments in their ability to function in the community.
  • Working knowledge of TAY case management services
  • Ability to respond quickly, effectively and with composure to member crises and life situations.
  • Professionalism, punctuality, flexibility and reliability are imperative
  • Excellent verbal, written, and interpersonal skills
  • Excellent organization skills and ability to multitask and juggle multiple priorities.
  • Outstanding ability to follow-through with tasks
  • Ability to work cooperatively and effectively as part of interdisciplinary team and independently assume responsibility
  • Strong initiative and enthusiasm and willingness to pitch in whenever needed
  • Ability to lead team members through verbal, written in-person directives
  • Able to communicate well at all levels of the organization including working with organization leadership and high-level representatives of partner organizations
  • Ability to work a flexible work week schedule; including at evenings and weekends

Desired:

  • Experience working within the –Department of Mental or Behavioral Health Services system
  • Knowledge of HMIS, Clarity, CES system
  • Verbal and written skills in one of the County’s threshold languages, including Spanish
