POSITION SUMMARY

Development and implementation of individual program plan in accordance with applicable state and federal regulations. Coordinate all habilitation direct care services as defined by State and Federal regulations provided to residents within the facility.

QUAIFICATIONS

The QDDP is a healthcare professional who possesses excellent written and oral communication skills, excellent organization and problem-solving skills. Should demonstrate an excellent ability to manage effectively, foster positive relationships, basic computer skills which are very essential to this position. Must possess a valid driver’s license.

EDUCATION

The QDDP must possess at least one year of documented experience working directly with individuals who have developmental disability or other developmental disability and also have one of the following criteria:

· A doctor of medicine or osteopathy licensed in Virginia

· A registered nurse licensed in Virginia or

· A Bachelor’s degree in human services or related field, including but not limited to sociology, social work, special education, rehabilitation counseling, or psychology.

RESPONSIBILITIES

1. Attend intake and ISP meetings in order to understand the individual’s support needs and be able to develop the individuals ISP based on available Part 3.

2. Develop and write Part Vs and upload them to Therap and WaMS.

3. Compile individual admission documents or annual ISP documents and ensure that they are signed on time.

4. Train staff on person centered support for each individual they are to support, and on Therap documentation.

5. Ensure that each trained staff meets competency prior to first day of work. i.e. completing competency checklist.

6. Completing a one-week review of staff performance, to ensure that supports are implemented as they should.

7. Complete an in person 30-day performance review of staffing support to ensure that services are implemented as they should.

8. Document all communication with staff and individual receiving services or authorized representative into Therap T logs.

9. Review documentation weekly and compile worked hours for payroll.

10. Review documentation for compliance with documentation standards and provide guidance to staff when needed for correction.

11. Submit payroll hours every two weeks, using accurate data collected from Therap.

12. Review documentation for completion of quarterly reports every quarter based on quarter report due dates. **All reports should be uploaded latest the 8th of the month.

13. While completing quarterly report, ensure that outcomes are being accomplished, and provide guidance to staff and support to the individual whenever outcomes are not being met.

14. Complete annual satisfaction survey for individuals

15. Complete annual staff evaluation along with annual competency checklist.

16. Conduct annual staff person centered training or whenever retraining is needed.

17. Properly document all training using appropriate sign in sheet and proof of training.

18. Coordinate efforts of the Interdisciplinary Team and program staff to ensure that each individual is receiving optimum services, which meet his/her individual needs.

19. Assist Administrator in coordinating all referrals, admissions, transfers, and dismissals of individuals from the facility.

20. Serves as a member of the quality assurance team.

21. Serve as the liaison with families/guardian, day training programs and the community.

22. Plan daily individual programs and activity schedules for all individuals in their caseload.

23. Participate/chair required committee meetings.

24. Perform other duties as requested by the Supervisor

25. Attend intake or individual annual meetings in-person if need be in the Virginia area.

JOB CONDITIONS

Minimal lifting of office records and printouts. Primarily, a desk job, which involves sitting, standing, stooping and walking. Excessive telephone communication and computer use. Traveling by car or airplane to local, out-of-town or state seminars, conferences or meetings. Must be able to read 12 point or larger type. Must be able to hear and speak in a manner understood by most people must be able to communicate both verbally and in writing. Job Type: Full-time

Pay: $21.00 - $27.00 per hour
