Elite Nursing Concierge Care is seeking compassionate, experienced Licensed Vocational Nurses (LVNs) to provide exceptional one-on-one care in private residences.
This is not a traditional home health position. Our clients expect personalized, concierge-level nursing delivered with professionalism, clinical excellence, and discretion. As an LVN, you will play an integral role in supporting clients through recovery, chronic disease management, and ongoing healthcare needs while working closely with our Registered Nurses and interdisciplinary care team.

What You'll Do
- Deliver skilled nursing care in private home settings under the direction of the supervising physician, in accordance with state regulations.
- Administer prescribed medications and treatments.
- Monitor, assess, and document patient condition, including vital signs and changes in status.
- Assist with activities of daily living, including bathing, grooming, dressing, and mobility assistance, as appropriate.
- Support post-operative recovery and ongoing chronic disease management.
- Reinforce patient education regarding medications, treatments, and physician-directed care plans.
- Communicate promptly with supervising clinicians regarding changes in patient condition.
- Collaborate with physicians, therapists, caregivers, and family members to ensure coordinated, client-centered care.
- Maintain accurate, timely, and compliant clinical documentation.
- Foster a safe, calm, and supportive environment while respecting each client's privacy and dignity.

Qualifications
- Current California Licensed Vocational Nurse (LVN) license in good standing.
- Current CPR/BLS certification.
- Minimum of 3 years of LVN experience
- At least 2 years of private-duty, concierge, or high acuity home care experience (required)
- Reliable transportation with the ability to travel to client homes.
- Strong clinical assessment, communication, and documentation skills.
- Ability to work independently while following established care plans.
Physical Requirements
- Ability to safely assist with patient mobility and transfers using proper body mechanics and assistive equipment.
- Ability to stand, walk, bend, kneel, and reach throughout the shift.
- Ability to lift, push, and pull equipment and supplies as required for patient care.
- Ability to respond quickly in emergency situations.
