The Opportunity
This is a full-time, permanent primary care role serving patients ages 2 and up within a collaborative, fully integrated care setting.
You will have the opportunity to build lasting relationships with your patients, practice comprehensive preventive care, and be part of a team that is genuinely changing lives in Central Georgia.
What You Will Do
- Provide primary care across the lifespan — pediatrics, adults, and geriatrics
- Diagnose and manage both acute and chronic conditions
- Lead preventive care, patient education, and chronic disease management
- Collaborate with an integrated team including physicians, behavioral health, and dental providers
- Deliver culturally competent, compassionate care to an underserved and diverse patient population

Qualifications:
- Board Certified Family Nurse Practitioner (FNP-BC or FNP-C)
- Active Georgia license or ability to obtain prior to start
- Primary care experience preferred
- Genuine passion for serving underserved and diverse populations
- Compassionate, patient-centered mindset and strong clinical judgment
