Registered Nurse / Director of Nursing - Home Health

We are seeking an experienced, high‐performing Registered Nurse who has served as a Director of Nursing in a Home Health Agency with deep expertise in home health operations, state surveys, and Medicare accreditation. The ideal candidate is a strategic clinical leader who brings passion, dedication, resilience, and the ability to elevate the agency through strong leadership and a commitment to excellence.

Key Responsibilities

• Lead all clinical operations of the home health agency with a focus on quality, compliance, and patient outcomes.
• Ensure full adherence to IDPH regulations, CMS Conditions of Participation, and Medicare accreditation standards.
• Direct and oversee preparation for state surveys, audits, and accreditation reviews, ensuring readiness at all times.
• Develop, Implement monitor clinical policies, procedures, and quality improvement initiatives.
• Prepare and submit Pre-Claim Review (PCR) under CMS's Review Choice Demonstration (RCD) program, required for Medicare home health agencies operating in Illinois."
• Serve as a coach and mentor, building a high‐performing, compassionate nursing team through training, accountability, and professional development.
• Provide on‐call clinical leadership to support staff, address urgent issues, and maintain continuity of care.
• Review and approve OASIS assessments, care plans, and clinical documentation for accuracy and regulatory compliance.
• Foster a culture of professionalism, teamwork, and patient‐centered care.
• Collaborate with administrative leadership to strengthen operations, improve efficiency, and support agency growth.
• Participate in case conferences, interdisciplinary coordination, and patient care planning.
• Address clinical issues promptly and maintain timely, compliant documentation.

Qualifications

• Registered Nurse (RN) licensed in Illinois (required).
• Minimum 3–5 years of home health experience (required).
• Prior experience as a Director of Nursing, Clinical Manager, or similar leadership role in home health.
• Strong knowledge of Medicare home health regulations, OASIS, IDPH survey processes, and accreditation standards.
• Demonstrated success leading teams through state surveys and Medicare accreditation.
• Experience with Pre-Claim Review (PCR) submission processes under CMS's Review Choice Demonstration (RCD) program, required for Medicare home health agencies operating in Illinois."
• Proven ability to motivate, coach, and develop staff into a high‐performing clinical team.
• Excellent leadership, communication, and organizational skills.

• Experience with EMR systems (Homecare Homebase, WellSky, or similar preferred).

Pay: $80,000.00 - $100,000.00 per year
