Job Overview
We are seeking a dedicated and compassionate Weekend Hospice RN Case Manager (RNCM) to join our Elevate Hospice team on a 7 on 7 off schedule for weekends. This position requires a schedule of 5pm Friday until 8am on Monday morning. In this role, you will provide high-quality nursing care to patients in a hospice setting, ensuring comfort and dignity during their end-of-life journey. You will collaborate with an interdisciplinary team to develop and implement individualized care plans, focusing on pain management and overall patient well-being.
Responsibilities
- Conduct comprehensive assessments of patients’ physical, emotional, and spiritual needs.
- Develop and implement personalized care plans in collaboration with the healthcare team.
- Administer medications and manage pain effectively to enhance patient comfort.
- Provide education and support to patients and families regarding disease processes, treatment options, and end-of-life care.
- Ensure adherence to established standards of care in outpatient settings.
- Coordinate with laboratory services and hospitals as needed for patient care.
- Maintain accurate documentation using electronic health record systems such as Athenahealth or Cerner.
- Perform airway management as necessary for patient safety.
- Advocate for the needs of patients and their families throughout the hospice process.
Experience
- Current RN license with experience in hospice or palliative care preferred.
- Familiarity with standards of care in outpatient settings is highly desirable.
- Experience in medication administration and pain management techniques is essential.
- Previous experience working with children in a healthcare setting is a plus.
- Strong communication skills and the ability to work collaboratively within an interdisciplinary team are required.
- Proficiency in using electronic health record systems such as Athenahealth or Cerner is advantageous.
