What are the responsibilities and job description for the Law Enforcement Liaison position at Elevate CCBHC?
Position Description: The Law Enforcement Liaison will be working with the Designated Collaborating Organization, however is still employed by Elevate Housing Foundation. This position is working with local law enforcement agencies as a crisis responder and will be assisting with mental health crisis calls along side law enforcement.
ESSENTIAL FUNCTIONS AND KEY RESPONSIBILITIES:
- Assist clients in accessing supports, services, and resources for their behavioral health recovery
Respond to 911 calls for assistance with support of Law Enforcement
Respond to calls for Mobile Crisis Services when available
Complete documentation in regards to any mobile crisis calls
Provide information and referrals for individuals seeking assistance
Provide crisis services, de-escalation, and follow-up for individuals per regulated timelines
Facilitate comprehensive habilitation enrollment and disenrollment processes
Develop and implement crisis plans/safety plans
Perform ongoing monitoring of the care plan to evaluate effectiveness, document interventions and goal achievement, and suggest changes accordingly
Promote integration of services for patients including behavioral health care and long-term services and supports, home and community to enhance the continuity of care for individuals
Assess for medical necessity and authorize all appropriate services
Conduct face-to-face or home follow-up visit when needed
Use motivational interviewing and clinical guideposts to educate, support and motivate change during individual contacts
Assess for barriers to care, provide care coordination and assistance to patients to address psycho/social, financial, and medical obstacles and concerns
Identify critical incidents and develop prevention plans to assure patient's health and welfare
