What are the responsibilities and job description for the Case Manager- ACT Team position at Elevate CCBHC?
Position Summary Elevate CCBHC is looking for a case manager to be part of the Assertive Community Treatment Team (ACT) in Southeast Iowa. The case manager will determine resources, and community supports that the individual may need. They will assure implementation of the treatment plan and that all identified needs are being met. The case manager will be doing intakes and evaluating ongoing needs for clients in the ACT program. This position will have some travel.
Job Description
- Participates in the assessment and development of the service plan.
- Develops and implements appropriate therapeutic activities and/or programs. Implements services to clients in accordance with their individualized plan.
- Collaborates with the client and client’s family or significant others to implement an effective service plan, explaining the available clinical options to the team, including the advantages and disadvantages of each option.
- Maintains the client’s comprehensive clinical record, including documentation of activities performed as part of the service delivery process (e.g., assessments, provision of services, coordination of care, discharge planning) and client’s progress toward their recovery.
- Works collaboratively with the clinical team to engage, educate, communicate, and coordinate care with client, their family, behavioral and general medical and dental health care providers, community resources and others to ensure that all services prescribed in the individualized service plan are implemented.
- Assures that discharge planning begins at admission. Obtains collateral input from providers and supports.
- Ensures all appropriate referrals for identified services on the service plan are made and coordinated with service providers via contracted network providers or community resources; Ensures all covered services identified on the service plan are evaluated and updated monthly.
- Ensures the development and implementation of transition, discharge, and aftercare plans prior to discontinuation of behavioral health services.
- Ensures all contact with referral source and/or other treatment providers is done within designated time frame and documented accordingly.
- Employs appropriate crisis intervention techniques as needed.
- Communicates appropriate client information to the treatment team; participates in team discussion and evaluation of client goals, objectives and motivations.
- Conveys medical criteria and clinical information between the insurance provider and treatment team as warranted
- Provide prompt screening and referral to appropriate treatment and supports.
- Assist with other resource/benefit acquisition and coordination as appropriate (Medicaid, Social Security, housing, social services, transportation, employment, medication assistance, etc) and develop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources.

- Provide community-based services to clients on education, community resources, community interaction and working on recovery plan goals.
- Completes all assessments and documentation as required by Elevate and required standards
Qualifications
- Bachelor’s degree in social work or other human services related field
- One year case management/care coordination experience
- 1-2 years providing support to individuals in behavioral health, recovery and mental health disorders.
- Valid IA drivers license
