Primary duties may include, but are not limited to:
Responsible for engaging with members to remove barriers to Social Determinants of Health (SDOH) and acting as the communication liaison between the company and referral sources.
Engage with members who have been identified as possessing significant SDOH barriers. Work with the member to establish goals and next steps for addressing SDOH.
Support member in selecting provider or programs most appropriate for supporting their needs. Interface with HCM to ensure that the individualized plan is aligned with existing or needed case management efforts.
Act as a liaison between referral sources and the health plan.
Associates in this role are expected to have the ability to multi-task, including handling calls, texts, facsimiles, and electronic queues, while simultaneously taking notes and speaking to customers.
Proficient in maintaining focus during extended periods of sitting and handling multiple tasks in a fast-paced, high-pressure environment; strong verbal and written communication skills, both with virtual and in-person interactions; attentive to details, critical thinker, and a problem-solver; demonstrates empathy and persistence to resolve caller issues completely; comfort and proficiency with digital tools and platforms to enhance productivity and minimize manual efforts.
Associates in this role will have a structured work schedule with occasional overtime or flexibility based on business needs, including the ability to work from the office as necessary.
Performs other duties as assigned.
Field (spends 4-5 days per week in-person with patients, members, or providers. This does not apply to on-site clinic personnel.)
Requirements:
Requires BA/BS degree and a minimum of 1 year of experience working directly with people related to the specific program population or other related community based organizations; or any combination of education and experience which would provide an equivalent background.
Onsite or in field 5 days/week. Confirmed for FTE at the end of the contract. OT approved as needed for roles within this health plan.

Additional Details:
Knowledge of Voc Rehab preferred.
M-F 8-5 co-located in the Onslow County DSS office. Need to live a reasonable commute distance to this office location. Onsite or in field 5 days/week. Confirmed for FTE at the end of the contract. OT approved as needed for roles within this health plan.
Travels to worksite and other locations as necessary.
Specific education and years and type of experience may be required based upon state law and contract requirements
Public Health, Social Work, Rehabilitation Counseling or related field and a minimum of 2 years experience with marketing, job development, job placement or social work; or any combination of education and experience which would provide an equivalent background
Elevance Health operates in a Hybrid Workforce Strategy. Unless specified as primarily virtual by the hiring manager, contractors are required to work at an Elevance Health location at least once per week, and potentially several times per week. Specific requirements and expectations for time onsite will be discussed as part of the hiring process. Candidates must reside within 50 miles or 1-hour commute each way of a relevant Elevance Health location.
