The use of this job is limited to Government Business Division (GBD) business units working on the Foster Care special program.

Responsible for engaging with community partners and builds strategy with local stakeholders; providing education, strengthening relationships, and helping resolve case or broader systems issues. Supports care coordination teams and connect members and caregivers with those who can provide insight and expertise to assist in overcoming challenges.

Primary duties may include, but are not limited to:

• Identify opportunities for engagement of members and their caregivers and families to share with care management team.

• Partners with peer support services in the community to create referral pathways and links members to appropriate community peer support services.

• Support care manager as they advocate for foster parents and other caregivers, addressing their needs or challenges with various agencies and systems.

• Develop strategy for peer support programs.
  
  

Requirements:

• Requires a HS diploma or equivalent and a minimum 2 years of related experience; or any combination of education and experience, which would provide an equivalent background.
  
  

Job Type: Possible contract to hire

Additional Details:

• Interpersonal skills, ability to work well with a team and computer literacy

• The location will need to be in NC within 50 miles of Rowan County, Stanly County, or Mecklenburg County, as these will be the areas the candidates will cover.

• Work schedule: Mon-Fri 8am to 5pm EST

• Face to face- wellness assessment

• Peer support

Elevance Health operates in a Hybrid Workforce Strategy. Unless specified as primarily virtual by the hiring manager, contractors are required to work at an Elevance Health location at least once per week, and potentially several times per week. Specific requirements and expectations for time onsite will be discussed as part of the hiring process. Candidates must be within a reasonable commute to a PulsePoint location.
