Elderwood is in growth mode and currently looking for a dedicated and compassionate LPN Case Manager to join our team! In this role, you will assist the Director of Nursing with coverage for critical staffing needs and work every other weekend. On call rotation is also required.
Ideal candidate will have experience with Assisted Living regulations along with prior supervisory experience.

Case Managers assist with ensuring the health and well-being of our residents by being responsible for the admission of residents; providing case management services for residents; and serving as the designated Discharge Planner for the facility.
Responsibilities
- Supervise/Administer and/or assist with medications to assigned residents; monitor monthly residents participating in self-medication program.
- Monitor the personal care provided by Resident Assistants.
- Assist Resident Assistants with direct care of residents, as necessary.
- Report to Director of Nursing change in condition, daily needs, and progress of residents.
- Follow the plan of care for each resident.
- Responsible for all aspects of receiving, storing and distributing controlled substance medications; ensure proper placement and use of medication stored by facility including med cart and medication room refrigerator.
- Maintain documentation regarding the provision of care to residents in the Case Notes, Medication Administration Records and other required records.
- Assist in problem-solving related to staffing, supplies and resident care during shift.
- Ensure that resident care standards are maintained in performance of duties, and that the rights of residents and preferences in care and treatment are respected.
- Receive and store lost/found articles and notify the Director of Nursing and/or Case Manager
- Ensure that residents are treated with respect and kindness at all times
- Coordinate the work of other case management and service providers within the facility.
- Conduct periodic rounds, provide input for the nursing care of residents, act as a nurse resource person for all nursing staff.
- Maintain safe conditions for all residents and staff.
- Monitor resident care standards during performance of duties and ensures that high standards of care are being provided.
- Participates in nursing on Call rotation of the facility.
- Assist Director of Nursing with additional duties and tasks as assigned.
- Complete Screening of prospective residents or residents returning to the facility from rehabilitation centers or hospitals to determine if the admission is appropriate.
Base wages increase with experience!
Qualifications
- Credentials as a Licensed Practical Nurse with a current State license required.
- Experience in geriatric nursing preferred.
- Organizational skills and ability to supervise department staff effectively and work well with personnel of other departments required; ensures high standards of care are maintained.
