The Case Manager Role Includes
- Coordination and management of patient care throughout the entire continuum, from admission through discharge and post-hospital follow-up.
- Collaborate closely with patients, families, physicians, interdisciplinary care teams, and third-party payers to ensure appropriate levels of care, seamless transitions, exceptional patient outcomes, and effective utilization of healthcare resources.
- Serve as a patient advocate while facilitating high-quality, efficient, and compassionate care delivery.

Required Education and Experience
License Requirement
- License to practice as a Registered Nurse in the State of New York.
- Education and Experience
- 1. Bachelor of Science Degree in Nursing and the equivalent of two (2) years of full time clinical experience in the field of Utilization Management/Discharge Planning, Home Health Care or Case Management; or
- 2. Associate’s degree in Nursing and the equivalent of four (4) years of full-time clinical experience in the field of Utilization Management/Discharge Planning, Home Health Care or Case Management; or
- 3. Nursing diploma and the equivalent of five (5) years of full-time clinical experience in the field of Utilization Management/Discharge Planning, Home Health Care or Case Management.
