Pay: $42–$55 per Billable Hour

About the Position

ElderCare Connections is seeking an experienced, dependable, and highly organized BSW or Registered Nurse to join our growing geriatric care management team.

We are a small, mission-driven company looking for someone who wants to build a long-term professional relationship with ElderCare Connections and grow with us. This position is best suited for someone who values meaningful work, enjoys being part of a close team, and wants to make a direct difference in the lives of older adults and their families.

The right candidate will be able to work independently, think critically, manage complex client situations, communicate professionally, and follow through without requiring constant direction.

This is not an occasional or one-day-per-week position. Applicants must have consistent availability at least three full days per week to support client continuity, appointments, follow-up, and time-sensitive care needs.

Position Overview

The Geriatric Care Manager will support older adults and their families through client visits, medical advocacy, care coordination, documentation, family communication, community referrals, and ongoing follow-up.

This is a hybrid role that includes remote and field-based responsibilities. Remote work may include documentation, telephone calls, emails, referrals, scheduling, provider communication, and family updates.

Field-based work may include client visits, medical appointments, facility meetings, hospital and rehabilitation visits, home visits, and client move coordination.

Key Responsibilities

Responsibilities may include:

  • Completing visits in client homes, hospitals, rehabilitation centers, assisted living communities, memory care settings, and skilled nursing facilities.
  • Attending medical appointments, care plan meetings, and discharge planning meetings.
  • Assessing client needs, safety concerns, changes in condition, and gaps in care.
  • Using critical thinking to identify priorities, appropriate next steps, and when a concern must be escalated.
  • Coordinating with physicians, facilities, home care agencies, home health providers, hospice teams, therapists, and community partners.
  • Communicating clearly with clients, families, providers, and ElderCare Connections leadership.
  • Completing timely, detailed, and professional documentation.
  • Coordinating referrals, services, appointments, care transitions, and client moves.
  • Tracking outstanding needs and ensuring follow-up is completed.
  • Building positive relationships with referral sources, providers, facilities, and community partners.
  • Supporting families through difficult aging-related decisions with compassion and professionalism.
  • Contributing ideas and solutions as ElderCare Connections continues to grow.

Schedule and Availability

Applicants must have consistent availability at least three full days per week. Exact hours and assignments will vary depending on client needs, appointments, referrals, travel, and caseload.

The availability requirement represents the ability to accept assignments and does not guarantee 24 billable hours each week. The caseload may begin gradually and increase as the company grows.

Some client or family communication may occur outside of traditional business hours. Contractors are not expected to remain continuously available but must provide reasonable responsiveness for accepted clients and communicate when backup support is needed.

Ideal Candidate

The ideal candidate will:

  • Hold, at a minimum, a BSW or an active Registered Nurse license.
  • Have experience working with older adults, families, or healthcare systems.
  • Demonstrate strong critical-thinking and problem-solving skills.
  • Be dependable, organized, proactive, and solutions-focused.
  • Work independently while remaining communicative and connected to the team.
  • Manage multiple client needs and competing priorities.
  • Communicate clearly and professionally in writing and verbally.
  • Complete documentation and follow-up in a timely manner.
  • Remain calm and professional during complicated or emotionally difficult situations.
  • Recognize changes in condition, safety concerns, and situations requiring escalation.
  • Be comfortable working with dementia, chronic illness, functional decline, and complex family dynamics.
  • Be willing to contribute to a small company and develop a long-term role as ElderCare Connections grows.

Experience in care management, case management, hospital discharge planning, home health, hospice, assisted living, memory care, skilled nursing, or community aging services is strongly preferred.

Travel Requirements

Reliable transportation and local travel are required. Service areas may include:

  • Fredericksburg
  • Stafford
  • Spotsylvania
  • King George
  • Bowling Green and Caroline County
  • Orange County
  • Other surrounding areas as agreed upon

Compensation

This is a 1099 independent contractor position paid at $48–$55 per approved billable hour, depending on experience, credentials, and fit.

Billable work may include client visits, appointments, documentation, family and provider communication, referrals, care coordination, client move support, and other approved client-related services.
