Key Responsibilities
● Overseeing Clientele that is assigned to your care
● Creating and maintaining relationships with our Caregivers and Nurses caring for your client
● Executing, implementing, and updating the Plan of Care for the client, ensuring the Caregiver is aware of changes
● Home visits - 75% of your time will be spent out in the field
● Onboarding prospect clients
● Monitoring Discharge from Hospital, SNF, or Rehab
● Completing administrative paperwork in accordance with our policy and DOH
Qualifications
● MA Registered Nurse with Up to Date License
● Ability to work autonomously
● Ability to work some weekends throughout the month
● Must feel comfortable clinically training RNs and Caregivers on-site to their respective duties for their clients
