Under direct supervision of the First 5 Program Manager, the SJ Teeth Care Coordinator will ensure Families and their children are linked to oral health servicesThe Teeth Care Coordinator supports the oral health initiatives of First 5 SJ by coordinating services, outreach, and education efforts aimed at improving dental health outcomes for children ages 0–5 and their families. This role serves as a liaison between families, dental providers, and community partners, ensuring access to preventive care, early intervention, and culturally responsive education.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Adheres to El Concilio California policies and procedures per the agency employee manual.
- Assist families with locating and enrolling for Oral/Dental Health services throughout San Joaquin County.
- Coordinate oral health services including screenings, fluoride varnish applications, and dental referrals
- Schedule and track dental appointments for enrolled children and follow up on missed visits
- Assist in developing and implementing oral health initiatives aligned with First 5 SJ goals
- Provide one-on-one guidance to families on oral hygiene practices, nutrition, and the importance of early dental care
- Conduct workshops, presentations, and outreach events for parents and caregivers
- Develop and distribute culturally and linguistically appropriate educational materials
- Build and maintain relationships with local dental providers, clinics, and community organizations
- Coordinate with healthcare providers, early childhood educators, and social service agencies to ensure integrated car
- Represent the organization at community meetings, health fairs, and collaborative networks
- Maintain accurate records of services, outcomes, and program participation in designated databases.
- Identify and engage eligible families through outreach strategies including community events and partner referrals
- Assist with program enrollment, eligibility screening, and intake processes
- Promote awareness of available oral health services within the community
- Track program metrics, including service utilization and health outcomes
- Prepare reports for internal evaluation and external funders
- Drop off learning and program materials to client’s residency
- Ensure compliance with confidentiality and data protection standards
- Required to work El Concilio California special events and meetings.
- Other duties as assigned by immediate supervisor and administration.

EDUCATION
- Associate’s or Bachelor’s degree in Public Health, Social Work, Health Education, or related field preferred.
- Minimum 1–2 years of experience in community health, early childhood programs, or non-profit services.

- Valid Driver’s License
- Up to date automobile insurance
- Successful completion of Sexual Harassment Prevention Training
- Successful completion of Workplace Violence Prevention Training
- Successful completion of HIPPA Training
DRIVING
Driving is an essential function of this position. All candidates must have:

- Proof of insurance and ownership for personal vehicles used during work duties
- The ability to qualify for the agency’s insurance coverage
- Required to work out of other El Concilio California offices and conduct outreach at public/community events.

The noise level in the work environment is usually quiet to moderate noise level. Primary work environment is an office setting, client residency and public venues.
