Position Details:
Eisner Health is seeking an experienced Optometrist with excellent communication skills to join our professional team and provide general optometry care to pediatric and adult patients.
Job Duties:
- Evaluation and treatment:
- Undertake medical evaluation, including history and physical examination.
- Develop diagnosis, treatment plan, and problem list.
- Initiate treatment orders, procedures, prescriptions, and referrals as appropriate.
- Educate and counsel patients to promote positive health behaviors and disease prevention.
- Work closely with clinical support staff in coordinating and obtaining needed services for patients and families.
- Continuing Care:
- The optometrist will make every reasonable effort to follow through personally with the patients s/he evaluated initially.
- Patients will be asked to return for further services only as often as necessary and by appointment whenever possible.
- Recording of Patient’s Progress, Procedures, and Other Matters:
- A visit will be recorded, and the chart note accepted in the EHR within three days of the encounter.
- Notes should:
- i. Reflect change in a patient’s condition and other significant information.
- Include documentation of pertinent history, review of systems, and physical exam
- Reflect provider’s thought processes as to the patient’s problems, diagnosis and treatment
- Document the need for continuing services and the appropriateness of diagnostic evaluations and treatment plans.
- Include referrals to specialty clinic services as determined to be medically necessary.
- Specify medications prescribed by the provider
- Reflect any pertinent attitudes or behavior on the part of the patient or family towards any aspect of care.
- Include notation of the need for related routine and preventive services such as screening tests related to chronic disease care
- Optometrist-patient communication:
- The patient shall be informed by the optometrist as to the general nature of his/her medical problem, the general prognosis, and the nature and purpose of the treatment and procedures that are contemplated. Appropriate instructions in self-care shall be given by the optometrist and/or the nursing staff to the patient and documented in the patient’s medical records.
- Optometrist interactions with patients and families are to be courteous and professional at all times.
- Productivity- Goal is for 14 patients to be seen in an average 8-hour day.
- Supervision- While in clinic, optometrist is expected to consult with supervisor as necessary. S/he may be asked to provide supervision and consultation to healthcare providers in training as needed.
- Optometrist is expected to use, protect, and disclose Eisner Health patients’ protected health information (PHI) only in accordance with HIPAA standards.
- Perform additional appropriate duties as assigned.
Qualifications:
- Must have a Doctor of Optometry degree and a current license to practice optometry in the State of California.
- Must have a current DEA License.
- Must be fluent in English with at least a working knowledge of medical Spanish preferred.
- Must possess current CPR Certification.
- Must be able to work in an interdisciplinary team setting.
- Must demonstrate effective communication and interpersonal skills with patients and families.
- Should have familiarity working with Electronic Medical Records.
