What are the responsibilities and job description for the MDS Nurse RN position at EHM Senior Solutions?
EHM Senior Solutions is excited to partner with a new MDS Nurse. The primary purpose of your job is to conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations and guidelines that govern the resident assessment, including the implementation for RAP's and Triggers.

Job Duties include, but are not limited to:
- Conduct comprehensive resident assessments using the Minimum Data Set (MDS) per CMS guidelines.
- Collect, review, and validate clinical data from multiple sources — nursing notes, therapy, dietary, and physician documentation.
- Develop and update individualized person-centered care plans based on MDS findings and resident needs while ensuring compliance with regulatory standards.
- Collaborate with nursing, therapy, social services, and dietary staff to implement care plans
- Maintain knowledge of current MDS criteria, ensuring assessments align with billing and reimbursement requirements.
- Monitor MDS transmission deadlines and ensure timely submission to CMS.
- Assist in preparing for state surveys and audits by maintaining accurate, compliant resident documentation.
- Monitor Quality Measures and provide feedback to the clinical team to drive continuous improvement.
- Educate nursing and interdisciplinary staff on MDS processes, documentation standards, and coding accuracy.
- Serve as a clinical resource for staff regarding assessment documentation, diagnosis coding, and CMS updates.
- Accurately code conditions, treatments, and services based on chart review and clinical guidelines.
- Active unencumbered RN License in the State of Michigan.
- 2 years of nursing experience in a skilled nursing facility
- 1 year of MDS experience in a skilled nursing facility
- Prior Knowledge of:
- Nursing practices and procedures
- Laws, regulations and guidelines that pertain to long-term care
- Practices and procedures surrounding MDS 3.0, PDPM, and Quality Measures
