Service Integration Coordinator / Community Health Worker Department: InCA
Minimum Requirements:
- Bachelor's degree in human service of realted field.
- Must have valid driver’s license and auto insurance
- Have excellent interpersonal skills, verbal and written communication skills.
- Preference will be given to candidates with experience working with clients with psychiatric or substance use issues and complicated medical needs.
- Must have ability to maintain confidentiality, comply with agency policies, and adhere to HIPAA standards.

Job Duties:
- Serve as the single point of service coordination contact for InCA designated Medicaid recipients age 21 and older through the iHUB.
- Conducts Comprehensive Needs Assessment and Screenings for all adult beneficiary referrals and walk-ins.
- Provides outreach and coordination contact for core partner agencies and InCA beneficiaries.
- Motivate clients to be active and engaged participants in their health.
- Provides accountability, support, motivation, and coaching to clients on improving health and wellness and learning appropriate self-care techniques.
- Coordinates and processes screenings for InCA beneficiaries.
- Supports and tracks linkages/referrals to community-based resources.
- Service Integration Coordinators will prompt, record and ensure care tasks within a Population Health Management system (i.e., follow-ups, wellness visits, screenings, care plans, etc.) are completed by appropriate direct service to improve health outco
