Home Health RN Department: Home Health
Minimum Requirements:
- Valid Illinois RN license
- One or more years of community health nursing experience.
- High energy level and empathetic ability in communication and human relations.
- Must have valid driver’s license, availability of transportation and must have auto insurance.

Job Duties:
- Is informed and involved with initial evaluation, start of frequency of visits and method of payment on all home health clients.
- Regularly re-evaluates the individual’s nursing needs.
- Is responsible for maintaining Home Health Program standards as outlined in the Rules and Regulations for Home Health agencies.
- May perform services in the homes of individuals within designated counties in accordance with the medical treatment plan for each individual patient made by the attending physician in conjunction with the EHD staff.
- Provides the following services: Evaluates the level of function, follows the plan of treatment, prepares clinical and progress notes, advises and consults with the family and other agency personnel and participates in in-service programs.
- Maintain an up-to-date clinical record for each patient receiving care.
- Provide progress notes to the patient’s physician about patients under care when the patient’s condition change or there are deviations from the plan of care or at least every sixty days.
- Individuals must demonstrate great cultural and socioeconomic sensitivity to populations served.
