Oasis Therapy Services is seeking a Physical Therapist to provide one-on-one rehabilitation services at Eden Vista Litchfield, an independent living, assisted living, and memory care community in Litchfield, Minnesota.

Responsibilities
- Evaluate residents and develop individualized physical therapy plans.
- Provide one-on-one treatment focused on strength, balance, gait, transfers, mobility, pain management, and fall prevention.
- Monitor residents’ progress and modify treatment plans as needed.
- Complete discharge planning and provide recommendations for continued care.
- Educate residents and caregivers on exercises, mobility strategies, and equipment.
- Direct and supervise Physical Therapist Assistants as appropriate.
- Complete accurate Net Health documentation and collaborate with the care team.
Qualifications
- Active and current Minnesota Physical Therapist license.
- Graduate of a CAPTE-accredited Physical Therapy program.
- Current CPR certification.
- Strong communication, organization, and documentation skills.
- Geriatric, independent living, assisted living, memory care, skilled nursing, or rehabilitation experience is preferred.
