Position Summary
The Director of Wellness RN / LPN oversees the day-to-day clinical operations of our assisted living and memory care community. This leader is responsible for ensuring high-quality resident care, supporting and developing clinical staff, and fostering a positive and organized care environment.
The ideal candidate is a strong communicator, an engaged leader, and a compassionate nurse who thrives in a collaborative setting and leads by example.
What You’ll Do
- Lead and oversee daily clinical operations to ensure exceptional resident care and regulatory compliance
- Supervise, coach, mentor, and support caregiving and clinical staff
- Partner with the Resident Care Coordinator(s) and leadership team to maintain strong clinical operations and staffing support
- Promote a positive, team-oriented culture focused on resident well-being and quality outcomes
- Support caregiver education through orientation, ongoing training, and clinical guidance
- Monitor resident health needs, changes in condition, and service plans while maintaining communication with physicians and families
- Ensure timely and accurate completion of nursing assessments, documentation, incident reports, and care plans
- Participate in resident evaluations, admissions, and transitions of care
- Collaborate with hospice, home health, and other healthcare partners to ensure continuity of care
- Lead or participate in resident care meetings and quality improvement initiatives
- Help create a calm, organized, and compassionate environment for residents, families, and staff
Qualifications
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Wisconsin
- Minimum of two (2) years of nursing experience, preferably in Assisted Living, Memory Care, Long-Term Care, or senior living
- Previous leadership or supervisory experience preferred
- Strong communication, leadership, and interpersonal skills
- Knowledge of geriatric nursing practices or willingness to learn
- Compassionate, dependable, and resident-focused approach to care
- Ability to maintain confidentiality and professionalism at all times

- Proficient in Microsoft Word, Excel, and email communication
- Excellent organizational and time-management skills
- Strong critical thinking and problem-solving abilities
- Ability to work collaboratively within an interdisciplinary team
- Demonstrates professionalism and excellent customer service
- Reliable attendance and flexibility to support operational needs
- Ability to sit, stand, walk, bend, lift, push, pull, and move throughout the workday
- Participation in on-call and Manager-on-Duty rotations as needed
