Day Habilitation Direct Support Professional Job Description
Qualifications:
1. High School Diploma or GED.
2. Experience working with individuals with developmental disabilities
preferred.
Driving Status: A valid driver’s license is required for this position
Reports To: Group Instructor/ Lead, QIDP, Operations Coordinator
Job Responsibilities:
1. Collaborate with all members of the interdisciplinary team to determine
suitable programming.
2. Demonstrate effective behavioral management techniques and procedures.
3. Implement person-centered plans by actively teaching life skills, socialization,
and daily living skills.
4. Provide assistance with personal hygiene and meals. Ensure that dietary,
medical, and personal choice needs are honored.
5. Record daily communication notes, track progress toward individual
habilitation goals using electronic databases as well as behavioral data both in
the community and at the program site.
6. Maintain a safe environment by adhering to OPWDD regulations, emergency
protocols, and specific behavioral or medical support plans.
7. Operate agency vehicles to safely transport individuals to and from their homes
and various community activities.
8. Facilitate and accompany individuals on community-based outings, volunteer
work, and recreational activities.
9. Additional responsibilities as determined by the Director of Long Island Adult
and Family Support Services.
Pay: $18.50 - $22.50 per hour
