This role requires a registered nurse with advanced knowledge and skills in patient assessment, critical thinking, and communication. You will work closely with the interdisciplinary team to develop and implement individualized care plans.
Key Responsibilities
- Perform comprehensive assessments and re-assessments of patients' conditions and care needs
- Develop and revise individualized plans of care based on assessment and patient response to treatment
- Evaluate patient responses to treatment and make adjustments as needed
- Collaborate with the healthcare team to promote quality improvement initiatives related to patient experience and safety
Requirements
- Graduate of an accredited school of Nursing with current licensure in the state of Connecticut
- Minimum one year of nursing experience required
- Able to communicate effectively in English both verbally and in writing
- Possesses basic computer skills and flexibility to float between Manchester Memorial and Rockville General Hospitals
