We are seeking a LPN- Licensed Practical Nurse to support our residents in our Skilled Nursing Facility (Transitional Care) in Burnsville, MN! This role performs a variety of professional nursing duties in compliance with the Comprehensive Home Care regulations and follows established policies and procedures.
This Nursing schedule includes;
- 32 hours every two weeks
- Day shift
- Includes every other weekend Saturday/Sunday and holiday rotations

Responsibilities:
- Provides skilled nursing care such as; implementing practitioners’ orders, ordering and assisting with medications and supplies, operates medical equipment, and charting practices
- Lifts and transfer residents as needed, Initiates emergency treatment as needed
- Monitors work and performance of unlicensed staff
- Updates RN with current observation and completes documentation of resident health status
- Communicates changes in residents conditions to relevant individuals
- May support RN by completing resident assessments, service agreements and care plans
Required:
- MN LPN licensure
- AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers
Preferred:
- Able to work independently
- Good communication and interpersonal skills
- Previous healthcare experience
- Comfortable using computers, hand held devices, phones and fax machine
