Ebenezer Integrated Care & Rehab (EICR) serves seniors with long-term, transitional, and dementia care needs in the heart of downtown St. Paul. As part of Fairview's Community Health and Wellness Hub, EICR is committed to delivering innovative, collaborative care. The Fairview Community Health and Wellness Hub opened in 2022 to deliver a variety of services to the community—from primary and mental health care to enrichment options for seniors, food access programs, and community gathering spaces. The Hub brings Fairview services together with local organizations—including Minnesota Community Care and Sanneh Foundation—to make it easier for people to access what they need to be well.
The Licensed Practical Nurse (LPN) provides for and assists in meeting the residents' physical, emotional, and psycho-social needs while adhering to all facility, state, and federal guidelines, Ebenezer Standards, and Policies.
This Nursing Schedule includes;
- 48 hours every two weeks ; part time
- Evening shift ; 2:00pm-10:30pm
- Block Schedule:
- Week 1: Wednesday and Friday
- Week 2: Monday, Friday, Saturday, and Sunday
- Transitional Care Unit
- Every other weekend and Holiday rotation
Responsibilities
- Monitors personal cares provided to assure consistency with each individual plan of care.
- Administers medications and performs treatments per physician's orders and facility policies.
- Assures that medical records and other documentation are current, complete, and accurate. Provides data relevant to the development of the plan of care.
Required Qualifications
- less than 1 year of experience
- Licensed Practical Nurse with the MN Board of Nursing
- Basic Life Support (American Heart Assoc, Red Cross or MTN)
Preferred Qualifications
- Associate of Arts
- 1 year of experience
