The Eau Claire County Case Worker- Recovery & Justice Services Liaison applies crisis intervention and case management expertise to support DHS 34 crisis services, address ongoing client needs, and assist with the Eau Claire Police Department’s Co-Response Program. This role requires strong assessment, coordination, and collaboration skills to work with individuals in the City of Eau Claire experiencing mental health or substance use concerns, while partnering across multiple systems to ensure effective, coordinated service delivery for Eau Claire County clients.

This is a second-shift position, with hours from 3:00 p.m. to 11:00 p.m. The position is part of the Eau Claire County Department of Human Services and is based at the Eau Claire Police Department.
  • Utilizes crisis assessment and intervention skills to conduct in-person and telephonic evaluations, effectively identifying needs and connecting individuals experiencing mental health concerns, substance use issues, homelessness, or trauma to appropriate resources and services.
  • Assist patrol officers in emergency mental health decision-making, follow-up/safety planning, maintain and navigate on-going relationships with high-utilizing community members. 
  • Provide on-scene assessments, offer recommendations on services, and follow-up contact to completed assessments, while documenting all activities per Eau Claire County policy. 
  • Applies collaborative decision-making and crisis response skills while partnering with law enforcement and the Co-Response Coordinator to assess situations and determine appropriate interventions.
  • Utilizes comprehensive assessment and case management skills to identify individual needs across medical, psychological, vocational, social, familial, and daily living domains.
  • Demonstrates service coordination and case management expertise by facilitating access to services, monitoring progress, ensuring timely delivery, and adjusting or closing services based on goal attainment.
  • Employs strong communication and education skills to inform and guide individuals and families on available community resources and how to independently access services.
  • Uses critical thinking and resource management skills to coordinate care effectively, avoid duplication of services, and maximize efficient use of staff time and available resources.
  • Maintains proficiency in documentation and compliance by adhering to record-keeping standards and legal requirements; accurately documents client interactions, manages case files and data, completes required reports, and prepares court-related documentation as needed.
  • Demonstrates commitment to professional development and team collaboration by actively participating in meetings, supervision, and ongoing training opportunities to enhance knowledge and skills.
  • Demonstrate effective case management by utilizing best practices and evidence-based methods to ensure timely, efficient, and appropriate service delivery.
  • Foster a positive team environment to support a deep collaborative culture within the multiple teams the role is involved in   
  • Performs other duties as assigned.
  • Bachelor’s degree in social work or a related field; and,
  • Minimum of 6 months of paid or unpaid experience in family, children, or adult case management services.
  • or successful completion of an internship in the specific area of hire
  • Must have DHS 34 Certification or obtain within three months of accepting position
  • Must have access to private transportation for work-related duties
  • Appointment will be conditional upon successful completion of criminal and caregiver background checks.
  • Must have valid driver’s license, an acceptable driving record, and access to private transportation for work-related duties.
  • Must possess and maintain personal automobile insurance in the amount of $100,000/$300,000.
