Case Manager/Community Support Specialist

Eastway Behavioral Healthcare is the largest, private, not-for-profit provider of mental health, substance use disorder, housing, residential, and employment services in the Miami Valley. We are now hiring for a dedicated and compassionate Case Manager/Community Support Specialist to work with a partner agency serving clients at a domestic violence shelter. This position will work with kids, women and families.

The Case Manager will support clients in the recovery process after being a victim of domestic violence. This will include facilitating Parenting and Family skills groups, individual case management services to clients, consultations and linkage to other services.

Schedule: Primarily Day Shift Monday – Friday; occasional evenings and weekends as needed.

Key Responsibilities:

· Collaboration with the partner agency.

· Provides trauma-informed and culturally aware services to family members and children admitted to the shelter.

· Facilitate group services tailored to participants’ ages and development level to promote understanding of trauma and support healing from its effects.

• Develops Individual Recovery Plans which include assertive interventions that directly target the improvement of the client's activities of daily living.
• Delivers individual services focused on improving functioning areas of need, including substance use concerns, mental health disorders, housing barriers, social and recreational concerns and safety or crisis situations.
• Timely completions of clinical documentation.
• Achieves agency productivity goals monthly.

Qualification

· Bachelor’s Degree in Social Work, Psychology, Counseling, or a related field.

· Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) preferred.

· Previous case worker or case management experience preferred.

· Prior experience working with individuals with addictions OR Chemical Dependency Counselor Assistant (CDCA) licensure preferred.

· Valid Ohio Driver’s License with proof of car insurance required as this position requires driving clients.

· Experienced in Windows-based computer applications

Pay: $19.00 - $22.00 per hour
