Essential Responsibilities
- Review, monitor and participate in the development, implementation and monitoring of program goals.
- Assist in the development of integrated, community-based employment and recreational activities, which reflect the individual’s interests, needs and goals/objectives.
- Assist in the preparation of documentation for treatment plan meetings and attend meetings as requested by Supervisor. Implement and monitor behavior plans.
- Assist with training and scheduling staff.
- Monitor the up keep of records, progress reports and daily/weekly documentation created by staff and provide corrective feedback.
- Perform Program Coordinator duties in the absence of the Program Coordinator.
- Perform direct support responsibilities as necessary including personal care and transportation.
- Provide services to individuals with varied behavioral issues which may include physical aggression, verbal assault, inappropriate sexual comments, gestures and /or acts.
- Work 1:1 with adolescent population.
- Support students in community and school settings.
- Travel state wide for student program coverage when necessary.
- Associates Degree in Human Services or related field, OR
- High School degree and two (2) years’ experience can be substituted for Associates Degree
- One year experience in Direct Support
- Valid driver’s license and reliable transportation
