Founded in 1970, East Valley Community Health Center is a Federally Qualified Health Center (FQHC) whose services include providing personalized, affordable, high-quality medical, dental, vision, and behavioral health care through a community-based network within the East San Gabriel Valley and Pomona Communities. Our staff practices patient-centered care by serving each patient with a personalized care plan that meets their individual needs. Our patients have access to support services that include, nutrition, health education, case management, pharmacy, lab, and x-ray at our health center locations. East Valley serves the healthcare needs of uninsured and underserved individuals and families throughout our 8 health center locations.

MAJOR POSITION RESPONSIBILITIES AND FUNCTIONS:
- Serve as the lead coordinator for Enrollment and Retention Specialists (ERS).
- Train and mentor new and existing ERSs to ensure they understand the ERS workflow.
- Extract and assign ERSs with monthly Med-Point Management call logs to drive Outreach and Retention.
- Monitor Call Logs and Event Tracker to ensure all activity is current and accurate.
- Maintain accurate inventory of office supplies, promotional items, and check-out list for department outreach equipment (Canopies, tents, tables, chairs, etc.)
- Liaison to our Health Plan Representatives and Insurance Brokers. Ensuring that ERSs are current with their knowledge of plan benefits; scheduling training as needed.
- Responsible for establishing trusting relationships with patients and their families while identifying, connecting, and supporting social determinants of health needs.
- Provide ongoing navigation with patients/families to assure patient satisfaction, evidenced by patient retention.
- Assist individuals and families with enrollment in health care coverage and facilitate “My Benefits” education sessions to ensure patients are knowledgeable regarding their health plan benefits.
- Ensure continuity of health care coverage, through ongoing tracking of patients’ redetermination dates, following up with patients via phone calls, home visits, and other necessary means.
- Increase East Valley penetration by identifying and reaching out to potential new patients residing in selected geographic areas.
- Create and maintain excellent working relations with community agencies/members, patients, visitors, and team members.
- Conduct in-reach activities at EVCHC sites to inform patients about healthcare coverage enrollment and/or re-enrollment.
- Participate in community events performing enrollment and access activities.
- Other duties as required by supervisor.
POSITION REQUIREMENTS AND QUALIFICATIONS:
- Bachelor’s degree in health administration, Business, or a related field or five years’ experience in a similar position.
- Minimum 2 years of progressive experience in outreach, enrollment, and patient retention for a healthcare facility, preferably with a federally funded health center conducting business in California.
- Experience working with disadvantaged populations and knowledge of health disparities are highly desired.
- Knowledgeable and/or certified in health care coverage systems such as Medi-Cal, Medicare, Covered California, and My Health LA program is a plus.
- Ability to think strategically and openly.
- Must have the ability to problem-solve effectively and meet deadlines.
- Must demonstrate integrity, sound judgment, leadership skills, and strong interpersonal skills.
- Excellent oral and written communication skills, and strong organizational, data entry, and documentation skills are required.
- Ability to handle complex or multi-faceted situations under pressure.
- Strong ability to multi-task and prioritize in a fast-paced environment.
- Must be accustomed to handling sensitive and private customer information.
- Must be comfortable coordinating community outreach activities and speaking to people in groups.
- Must have strong Microsoft Office skills (Outlook, Word, and Excel).
- Must be able to type 45 words per minute.
- Bilingual, Fluent in English/Spanish or English/Mandarin.
DOE: $28.00 - $33.06
