Qualifications/Job summary: The Licensed Practical Nurse (LPN) provides clinical oversight, direct participant care, and health monitoring within an adult day care setting serving older adults, individuals with chronic illnesses, dementia, psychiatric conditions, and VA participants. The LPN ensures a safe, supportive environment and collaborates with families, providers, and interdisciplinary staff to promote participant well‑being.
RESPONSIBILITIES/DUTIES· Administer oral, topical, inhaled, and injectable medications within LPN scope.· Perform treatments such as wound care, glucose monitoring, oxygen management, and catheter care.· Maintain medication inventory, storage compliance, and controlled substance logs
- Perform daily health assessments, including vital signs, symptom checks, and monitoring for changes in condition.
- Provide first aid and respond to medical emergencies according to center protocols.
- Administer medications per physician orders and maintain accurate MAR documentation.
- Monitor participants for adverse reactions, behavioral changes, pain, or decline.
- Manage chronic conditions such as diabetes, hypertension, COPD, CHF, dementia, and psychiatric diagnoses.
- Conduct fall assessments, skin checks, and safety evaluations.
· Requires frequent walking, standing, and assisting with mobility.
· Involves interaction with families, VA case managers, social workers, and community providers.
- Complete nursing notes, incident reports, and care plan updates in accordance with peer review and CT DPH standards.
- Document all interventions, observations, and communications in a timely and accurate manner.
- Communicate significant changes to families, caregivers, VA providers, PCPs, and internal leadership.
- Participate in interdisciplinary meetings in RNs absence and contribute to individualized care plans.
- Promote a safe environment by monitoring mobility, transfers, and fall‑risk behaviors.
· Support participants with ADLs as needed (toileting, hygiene, ambulation).
· Observe and report behavioral health concerns, including agitation, depression, or confusion.
· Implement infection control practices and monitor for signs of illness.
· Strong assessment, communication, and documentation skills.
· Ability to work independently while collaborating with a multidisciplinary team.
· Compassion, patience, and professionalism when working with diverse populations.
· Ability to work with older adults, dementia care, psychiatric care, or adult day services
Pay: $29.00 - $30.00 per hour
