Position Summary:
We are seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our interdisciplinary healthcare team. The ideal candidate will provide behavioral health services to patients of all ages in a primary care setting, working closely with medical providers to develop and implement integrated care plans.
Key Responsibilities:
- Provide individual, family, and group therapy using evidence-based practices
- Conduct comprehensive assessments, diagnose mental health conditions, and develop treatment plans
- Collaborate with physicians, nurses, and other staff as part of an integrated care model
- Provide crisis intervention and support as needed
- Maintain accurate and timely documentation in the electronic health record (EHR)
- Assist patients in accessing community resources and referrals
- Participate in case consultations, quality improvement, and team meetings
- Ensure compliance with HIPAA and other regulatory standards
Qualifications:
- Master’s degree in Social Work from an accredited institution
- Licensed Clinical Social Worker (LCSW) in the state of Georgia
- Minimum 2 years of clinical experience preferred (FQHC or primary care experience a plus)
- Strong understanding of trauma-informed care, cultural competency, and integrated behavioral health
- Excellent communication, organizational, and interpersonal skills
- Bilingual in Spanish a plus but not required
