Responsibilities
- Face-to-face & telephonic communication
- Perform phone triage, patient education and coaching
- Use of disease management interventions to engage patients in their health care
- e-prescribe
- Immunizations
- EMR experience
- Vital signs
- Medication reconciliation
- Specimen collection with in-house testing
- EKGs and spirometry
- Participate in the development and implementation of individualized care plans.
Skills
- Excellent communication skills for effective interaction with patients, families, and team members.
- Ability to work collaboratively within a team environment.
- Strong organizational skills with attention to detail in clinical documentation.
