Are you passionate about the senior population and making a real difference in their lives? EAC Network, a leading not-for-profit social service agency dedicated to helping over 79,000 people through 100 impactful programs across Long Island and NYC, is looking for a part time Home Assessor to join the Respite Companion program.
- Salary: $24 per hour
- Schedule: 20-29 hours per week
- Location: 99 Quentin Roosevelt Blvd in Garden City, NY (This role will be remote but local to Nassau County).
What drives the core purpose of this job:
The Home Assessor is responsible for conducting home visits to assess the health and mental status of the caregiver and care recipient to determine eligibility for the In-home Respite Services Program.
What are the key responsibilities:
- Conduct home visits to assess the health and mental status of clients to determine eligibility for the Senior Respite Program.
- Develop a care plan that determines the level of supervision and support for the care recipient.
- Identify the need for additional services and make necessary client referrals to those services.
- Record all client and related information on a laptop computer.
What key knowledge, skills, and abilities are required to excel in this role:
- Must be a Registered Nurse, Licensed Practical Nurse or have a minimum of a Bachelors degree in social work or human services.
- Minimum of two years of experience required working with the geriatric population.
- Ability to communicate with and understand the needs of the elderly.
- Must possess a thorough knowledge of Nassau County services for the elderly.
- Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, email and/or database software necessary to perform work assignments.
- Ability to utilize a laptop computer required.
- Must have access to automobile and possess valid driver's license to operate in NY State.
