Schedule:

• Initial: Monday–Friday, 1:00pm–7:00pm
• After 90 days: Monday–Friday, 9:00am–4:00pm (7 hours/day)

Position Details:
We are seeking a dependable and compassionate Direct Support Professional to support a young woman as she transitions from a group home setting back into her family’s residence. During the first 90 days, the client will attend an Intensive Outpatient Program (IOP), with the schedule set for Monday–Friday, 1:00pm–7:00pm. After discharge from IOP, the schedule will shift to Monday–Friday, 9:00am–4:00pm.

This role involves close collaboration with the individual and her family to develop and implement a personalized care routine that promotes safety, comfort, and independence at home and in the community. The ideal candidate will be patient, flexible with scheduling, communicative, and committed to fostering a supportive home environment during this period of adjustment.

Key Responsibilities:

• Support with daily living tasks and foster independence
• Encourage community involvement and social engagement
• Promote personal growth through skill development and positive reinforcement
• Provide compassionate care in home and assisted living environments
• Assist with ADLs, including meal preparation, household maintenance, and mobility

Qualifications:

• High school diploma or equivalent
• Patient, punctual, and dependable
• 2 years’ prior experience supporting individuals with physical disabilities preferred
• Adult First Aid/CPR certification
