Responsibilities
- Conduct comprehensive patient assessments to determine physical therapy needs, utilizing your understanding of physiology, anatomy, and medical terminology.
- Develop and implement tailored care plans that incorporate therapeutic exercises, manual therapy techniques such as trigger point therapy, and modalities suited to each patient's condition.
- Manage discharge planning processes by coordinating with multidisciplinary teams to ensure seamless transitions from hospital to home or community care settings.
- Provide specialized care for diverse populations including pediatrics, geriatrics, orthopedics, and patients in acute care or outpatient clinics.
- Educate patients and their families on injury prevention, treatment strategies, and home exercise programs to promote ongoing recovery outside clinical settings.
- Document all patient interactions accurately within electronic medical records (EMR systems) while maintaining compliance with healthcare regulations.
- Collaborate with healthcare professionals to refine treatment approaches and optimize patient outcomes in inpatient, outpatient, or home health environments.
Requirements
- Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.
- Proven experience in inpatient hospital settings, outpatient clinics, or home health environments demonstrating proficiency in physiotherapy techniques.
- Strong knowledge of physiology, anatomy, medical terminology, and therapeutic modalities including trigger point therapy.
- Experience with discharge planning and creating effective care plans tailored to individual patient needs.
- Familiarity with EMR systems for documentation and patient record management.
- Excellent patient care skills with the ability to communicate complex information clearly and empathetically.
- Background working with diverse populations such as pediatrics, geriatrics, orthopedics, or patients requiring acute care services is highly desirable.
- Home & community care experience is a plus for providing holistic support beyond clinical settings.
