Social Services Assistant
Job Summary:
We are seeking a dynamic and experienced Social Services Assistant to join our team at Dubois Continuum of Care Community in DuBois, Pennsylvania. As a Social Services Assistant, you will play a vital role in identifying and meeting the medically related emotional and social needs of our residents and their families. With a focus on providing exceptional care and support, you will be responsible for ensuring that our residents receive comprehensive services that promote their overall well-being.
Responsibilities:
Ensure that the emotional and social needs of our residents are met and maintained on a daily basis
Provide discharge planning services for residents, arranging appropriate referrals
Provide information to residents and families regarding financial assistance programs (e.g. Medicare/Medicaid)
Interview residents and families as necessary to obtain social history and other needed information
Develop and revise care plans
Attend interdisciplinary care plan meetings
Act as an advocate for residents and report violations of residents' rights to the Director of Social Services
Complete insurance updates
Follow direction of the Social Services Director and complete tasks assigned by them
Requirements:
A Bachelor's degree in Social Work, Psychology, human servieces, or a related field
0 years of minimum experience in senior care or community-based organizations required
Ability to meet facility physical capability standards for the position
