Licensed Independent Social Worker

Pay

  • 40 per hour
  • 60 OT
  • 1729 weekly per diem
  • 500 travel allowance
  • One year with four option years

Schedule

  • Monday through Friday
  • 8am to 4:30pm
  • No overtime approved under the contract
  • Santa Clara clinics two to three days each month
  • Santa Fe clinics all remaining days
  • Coverage at Cochiti and San Felipe as needed

Role

  • Provide counseling to individuals and families
  • Deliver crisis intervention, diagnosis, and care coordination
  • Support adolescent, adult, and geriatric patients
  • Complete assessments and follow-up plans
  • Participate in treatment planning with the Mental Health team
  • Coordinate referrals including PRC services
  • Consult with physicians, psychiatrists, and clinical staff
  • Document all care in the EHR
  • Follow IHS, CMS, HIPAA, and AAAHC requirements
  • Join quality improvement and peer review activities
  • Attend department meetings

Requirements

  • BSW from a CSWE-accredited program
  • MSW required for independent licensure
  • Active LISW or LCSW
  • Current BLS
  • ACLS or PALS preferred
  • Past performance showing two social worker placements within the past two years

  • San Felipe Health Clinic, San Felipe Pueblo NM
  • Santa Clara Health Center, Espanola NM
