Duties
- Assess patient needs through comprehensive intake interviews and medical record reviews, including EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems.
- Develop individualized care plans that incorporate medical, social, behavioral health, and community resources to promote optimal recovery and well-being.
- Coordinate with multidisciplinary teams—including nursing, social work, physicians, and specialists—to facilitate timely interventions and treatments.
- Manage discharge planning processes by evaluating post-hospital needs, arranging home health services, hospice care, or rehabilitation as appropriate.
- Conduct ongoing case reviews to monitor progress, adjust care plans, and ensure compliance with HIPAA regulations and healthcare standards.
- Serve as a primary point of contact for patients and their families, providing clear communication about treatment options, insurance coverage, and available social services.
- Collaborate with managed care organizations and health insurance providers to verify benefits and facilitate utilization management.
Skills
- Strong nursing background with knowledge of medical terminology, CPT coding, ICD coding (ICD-9/10), and discharge planning procedures.
- Knowledge of HIPAA regulations, health insurance processes, managed care protocols, and utilization management practices.
- Excellent client communication skills with the ability to work effectively with individuals from diverse cultural backgrounds.
- Background in social work or behavioral health enhances the capacity to address complex psychosocial needs related to patient care.
- Ability to handle intake assessments efficiently while demonstrating empathy and professionalism throughout interactions.
