We are looking for a Fulltime SASH Coordinator to join our team!
Downstreet is seeking an enthusiastic and dedicated fulltime SASH Coordinator to help Montpelier & Northfield residents age safely at home, meet basic needs, achieve personal stability, and thrive. This person will provide direct services and connections to community resources.
Key Responsibilities
- Identify needs of individuals and develop healthy living plans to meet those needs. Develops and maintains a regular Community Health Living Plan.
- Connect participants to community resources and services, and follow-up on engagement and outcomes.
- Coordinate opportunities for participants to develop social connections and build community.
- Thoroughly explain the Use and Disclosure Agreement and related documents to participants and their family members/supports.
- Adheres to confidentiality guidelines and appropriate documents information releases.
- Establish connections and maintain strong working relationships with local service providers. Represent Downstreet and SASH at relevant community meetings.
- Maintain collaborative relationships with participants, Downstreet staff, and community partners. Including the ability to work individually and as part of a team.
- Complete all administrative duties related to the position, including participant files, program monitoring, and reporting in a timely manner.
- Ensure compliance with all relevant legal, regulatory, and organizational requirements.
Experience and Education
- Bachelor’s degree and 3 years of experience in social work, or an equivalent combination of education, employment, and life experience
- Previous experience coordinating services specifically for older adults and/or people with disabilities is preferred
- Proficient in Microsoft Office Suite and ability to learn relevant property management software
- Highly organized with a strong ability to manage priorities and deadlines and ability to manage confidential information
- Strong written and verbal communication skills, active listening skills, and problem-solving abilities
- Must have knowledge of and appreciation for individual autonomy and choices
- Commitment to equity and inclusion, and social and economic justice
- Knowledge of area resources and programs available to older adults and people with disabilities is strongly preferred
- Valid Vermont Driver’s License and reliable personal vehicle
Physical Requirements
- Prolonged periods sitting at a desk and working at a computer
- Must be able to lift 25 pounds at a time
- Ability to access all Downstreet sites, buildings, apartments, and facilities
- Must be adaptable to working in an ever-changing, high pressure work environment

Salary: $50,000/ann.
