Description:Position Summary: The Quality Assurance and Performance Improvement (QAPI) Nurse serves as the clinical leader for quality and performance improvement across all Dover Health Home Health and Hospice agencies. This position plays a critical role in promoting data-driven quality oversight, ensuring regulatory compliance, and advancing patient safety initiatives. The QAPI Nurse is responsible for the development, coordination, and ongoing evaluation of quality assurance and performance improvement (QAPI) programs to support the consistent delivery of safe, effective, and compliantcare across all service lines.In collaboration with clinical and operational leadership, the QAPI Nurse monitors quality trends, evaluates risks, and directs targeted improvement initiatives aligned with CMS QAPI standards and organizational objectives. Through comprehensive data analysis, documentation review, staffeducation, and regulatory preparedness efforts, this role fosters a culture of accountability,continuous improvement, and excellence in patient outcomes within Dover Health’s Home Health and Hospice programs.Essential Duties: The following duties are normal for this position. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Collect, review, and analyze quality and performance data across Home Health and Hospice agencies. Monitor and trend clinical outcomes and CMS quality measures, patient and family satisfaction survey results, infection prevention and control data, and incident reports, adverse events, and near-miss events. Identify trends, risks, and opportunities for improvement and escalate findings as appropriate. Lead assigned Performance Improvement Projects (PIPs) using CMS QAPI methodology. Develop action plans to address identified quality gaps (e.g., falls, medication management, documentation accuracy). Monitor the effectiveness of interventions and ensure sustainability of improvements.Support compliance with CMS Conditions of Participation for both Home Health and Hospice, including QAPI requirements. Assist with ongoing audit and survey readiness, documentation reviews, and corrective action follow-up. Stay current on regulatory changes impacting quality, patient safety, and clinical documentation. Conduct targeted chart audits to ensure completeness, accuracy, and regulatory compliance. Provide feedback and education to clinicians and leadership on documentation best practices. Develop and deliver education related to quality initiatives, regulatory requirements, and best practices. Partner with agency leadership to reinforce standards and promote consistent clinical practices across agencies. Monitor adverse events, near misses, and infection trends. Collaborate with leadership to implement preventive strategies and reduce risk across service lines. Prepare and present quality reports, dashboards, and improvement updates at QAPI and leadership meetings. Communicate findings and recommendations clearly to clinical and operational leaders. Other duties as assigned.Qualifications, Education, and/or Experience:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and abilities required. Active Registered Nurse (RN) license in Missouri and Illinois, or the ability to obtain licensure in both states, is required. A minimum of five (5) years of clinical nursing experience is required. Clinical experience in Home Health and Hospice settings is required. A bachelor’s degree in nursing (BSN) is preferred. Certification as a Certified Professional in Healthcare Quality (CPHQ) or similar qualityfocused credential is preferred. Experience with Quality Assurance and Performance Improvement (QAPI) programs, quality audits, or performance improvement initiatives is preferred. Familiarity with CMS Conditions of Participation for Home Health and Hospice is preferred. Strong analytical, critical thinking, written, and verbal communication skills, with the ability to interpret clinical and quality data and apply findings to quality improvement efforts, are required. Proficiency with electronic medical records (EMRs) and quality reporting tools is required; experience with Netsmart is preferred. Strong organizational skills, attention to detail, and the ability to manage multiple priorities while working collaboratively with clinical and operational leadership across multiple agencies are required. The role involves working at a desk for most of the workday. This position may need to move through areas of the corporate office or other worksites ofthe organization.This position requires regular travel (up to 25%), as dictated by business needs. This position involves juggling multiple projects simultaneously, which requires strong time management and organizational skills. There may be deadlines for hiring specific roles or initiatives.This role involves actively participating in all staff activities aimed at fostering teamwork, unity, and morale. The individual will contribute as a collaborative team player, working alongside colleagues to create a supportive and cohesive work environment. Individuals in this position are required to stay current on all training and ongoing education initiatives.
